Steps to Self Management plus
with Homecare My Way
1 Design your care plan
We will work with you to design a personalised care plan. This serves as an essential guide for you and your care manager when setting up your initial care and services.
2 We’ll search for care and services
Using your care plan, your care manager will search for services best suited for your needs, liaising with you throughout the process. We will perform all necessary checks on the service providers such as insurance, compliance, and qualifications to ensure they meet our high standards.
3 Setting up your care and services
Once the necessary checks are completed, your care manager will set up your services on the schedule outlined in your care plan. Once set up, the ongoing management and scheduling of these services will be handed over to you.
4 Complete your budget
While your services are being set up, your care manager will work with you to complete your budget so you fully understand where your homecare package funds are being spent.
5 Find additional service providers
If you want to find a different service provider, your care manager will take back the reins and find new service providers at your request.
6 Update us on any changes
It is crucial that you inform us if your needs change, especially if there is a change in your health or circumstances. Homecare My Way will work with you to continually review and adjust your care and services to match your health needs.
